Choices NZ works with you to strive for greater independence in how you choose to participate day-to-day in your community. We will assist you to plan everyday interactions, providing coaching and developing strategies with you to connect with people and organisations in your community so you can achieve your personal, community and employment goals. Choices NZ gives you confidence to take control of the everyday aspects of your life – ‘where I live, what I do, where I go and who I’m with’.
The service consists of four components:
- Facilitated pathway map
- Independent facilitation
- Tailored supports (including individual and group supports)
- Coaching to self-manage
Who is eligible?
You are eligible if you:
- Have a significant impairment, complex needs or disability including mental injury
- Have an accepted claim with ACC for this injury
- Are assessed and referred by ACC as benefiting from this service

This service is available nationwide.
